The preferred method of communication is email, and we offer an encrypted option for your privacy on our Contact page. If you prefer to call, simply call the number listed on the Contact page and leave a message on our confidential answering system and someone will return your call as soon as possible.

The next step is the completion of paperwork. As with any other physician or care provider, there are forms that must be completed prior to the first visit with facilitate smooth service on the behalf of your provider's office. To assist the staff and expedite service, you can download these forms at our convenience and then bring the completed paperwork to your first appointment. These forms can be found under the New Patient tab on the Forms page.
If you are filing insurance, please bring your insurance card and ID so that we may obtain a copy.
Cancellation Policy - The policy states that any changes to scheduled appointments should be made at least 24 hours in advance. Missed appointments which are not properly cancelled are subject to a missed appointment fee, which can be equal to but not exceed the therapist's regular appointment fee. Insurance companies do not pay missed appointment fees and the patient/responsible party is held fully accountable for the charge. In addition, if you schedule an appointment as a new patient and cancel/reschedule three times before becoming an established patient at our office we will offer you referral information for other local offices that may better suit your scheduling needs.
